Discuss your concerns and desired outcome
Assessment of your current eating habits and familial history
Go over a path, that will best meet your needs
Health Consultation $129: (If you choose the 3 month or 6 month “health plan” then consultation is $75)
Evaluation for nutritional deficiencies
Help determining, any possible food intolerance’s
Discuss important self-help remedies, through the therapeutic use of food and supplements
A health questionnaire(s) and consultation agreement will be sent to you to be filled out and returned, prior to your appointment. This can be emailed or sent regular mail.
Individual Health Path:
3 Month Plan $160 + $75 (Discounted Health Consultation) = $235
- (4) – 30 minute follow up consults: Done every 3 weeks, to discuss progress, evaluate plan and answer questions
- (1) – Weekly email from me to see how you are going, address 1 or 2 questions/concerns
6 Month Plan $280 + $75 (Discounted Health Consultation) = $355
- (8) – 30 minute follow up consults: Done every 3 weeks, to discuss progress, evaluate plan and answer questions
- (1) – Weekly email from me to see how you are doing, address 1 or 2 questions/concerns
Living in the Health Zone Group Path:
I waive the fees, to those that become home direct Shakeology customers. Fee can also be waived for 30 Day, by purchasing one of the Beachbody workout programs. Contact me for more information.
